Quite often family members live out of state or are working and want an extra set of eyes on their parents or loved ones. We have Geriatric Care Managers available to provide our clients and families with that extra peace of mind. Having a skilled medical professional available to ensure your loved one is getting the care they deserve can help relieve stress on other family members.  It also allows the client to feel they are still independent but have the help of a trusted friend for support instead of feeling like a burden on their children or families.

Care Management Services include but are not limited to the following:

• Attending Physician Appointments (reporting information back to family members if needed)
• Budget Planning or assistance with Paying Bills
• Care Coordination
• Medication Management
• Patient Advocacy (Includes Physicians appointments, ALF, Hospitals or Nursing Homes)
